The McCormack Fitness Center is multi-agency facility offering fitness and workout classes to Federal, State and City employees.

2019 Annual Membership
Fee Schedule

Membership is from 1/1/2019 to 12/31/2019.

Membership Rates are as follows:

  1. $180 - January 1st - December 31st
  2. $135 - April 1st - December 31st
  3. $90 - July 1st - December 31st
  4. $60 - September 1st - December 31st

Renewal Notice

All members will need to fill out the MFC 2019 membership form.

The membership form, including the Health Questionnaire, can now be completed and submitted electronically. Please fill it out completely, and either submit it electronically - using the Submit button at the end of the application - or print and return the form and applicable payment to any Board Member or Dr. Bruce Cohen. Applications are due by December 31, 2018 to avoid having your Smartcard deactivated.

Note: Dr. Bruce's schedule is Tuesdays 10:30 am - 2:30 pm

Please mark your applications accordingly at the bottom of the Personal Information Form section.

For people needing a signed Physician's Form, if you have one on file and need another note according to this year's Health History Questionnaire, you have until March 1, 2019 to get a new note to MFC, Inc. For people needing a note for the first time, a doctor's note needs to be on file before we can grant you access (i.e. by 12/31/2018). Finally, for those that recently provided a doctor's note since July 1, 2018 (only applies to a few people), no doctor's note will be needed for the one-year 2019 membership.
